Transitional Support
We assist with essential, one-time expenses necessary to help you transition into stable
housing-such as deposits, furnishings, initial rent payments, and utility set-up costs.
Relocation Assistance
Our team provides hands-on guidance throughout your move. We support you in understanding
lease agreements, setting up utilities, and ensuring a smooth transition into your new home.
Moving Home Minnesota Support
For individuals moving from care facilities or institutional settings back into the community, we help coordinate housing placement, service navigation, and ongoing stability planning.
Community Support Services
We offer intensive, short-term support during challenging moments-providing crisis navigation, mediation, resource planning, discharge coordination, and homelessness prevention services.
